Minors must have valid work permits and Coogans. RATES: *+20% Agency Fee if applicable SESSION FEE: $500+20% (8 hour session for adults, max time on set given age for minors). ADULT USAGE FEE: $2,500+20% to include the below term, territory, and media. Guaranteed at time of booking. MINOR USAGE FEE: $1,500 + 20% to include the below term, territory, and media. Payable only if used in final edits. **All talent fees will be paid via Extreme Reach USAGE 3-year USA unlimited consumer + HCP disease state awareness, print + digital use. Cost includes but is not limited to social (paid + unpaid), website, banners, emails. Usage to begin date of first distribution. Cost includes all outtakes, stills and motion. Broadcast and OOH are excluded. ADDITIONAL USE OPTIONS AND FEES: 4th Year Use Adult = $850 + 20% agent fee 4th Year Use Minor = $550 + 20% agent fee 5th Year Use Adult = $935 + 20% agent fee 5th Year Use Minor = $605 + 20% agent fee
