Preston Grey is undergoing sexual response testing. Despite being young, wealthy, powerful, and conventionally attractive, Preston suffers from complete erectile dysfunction. Doctors cycle him through stimuli meant to provoke arousal, but nothing works. The scene establishes Preston’s internal conflict — rage, shame, and a compulsive need for control — as well as the mystery of his condition. He leaves the appointment furious and defensive. Preston is the President of Grey Medical Group, a vast hospital empire founded by his grandfather Richard Grey, a domineering patriarch who believes legacy is everything. Preston’s inheritance is conditional: the first grandson to produce an heir will inherit the company. Preston’s brother Owen Grey is irresponsible but fertile, and already positioning himself to win. Preston’s condition therefore threatens not just his masculinity, but his entire future. Preston’s closest confidant, George, understands Preston’s emotional detachment and dry cruelty better than anyone. George pushes Preston to confront the possibility that his condition is psychological, not physical. Doctors echo this, suggesting that Preston may need to relinquish control — even submit — to resolve his block. The idea enrages Preston. In parallel, Alexandra “Alex” Bailey, is a medical intern, intelligent and hardworking, but visibly exhausted. Alex’s life revolves around survival. She was raised by her grandmother Ellis, who is now seriously ill and accruing massive medical bills. To pay them, Alex works nights at a bar dressed in a Playboy-bunny costume. The contrast between Alex’s compassion and her degrading circumstances defines her character. One night at the bar, Preston sits in a private room with George and Izzie Karev, a wealthy, aggressive woman who openly pursues Preston. He rebuffs her advances with cold contempt. Alex enters to serve drinks. She trips in her heels and accidentally falls into Preston’s lap, instinctively grabbing his thigh to steady herself. The contact triggers an immediate physical response in Preston — the first he has ever experienced. Both freeze. Preston is stunned, shaken, and visibly affected. Alex is mortified and apologetic. The moment is interrupted when Izzie recognizes Alex. She reveals that Alex is the granddaughter of the woman she once struck with her car and abandoned at the scene. Tension escalates as Izzie insults and threatens Alex. Preston intervenes, dismissing Alex abruptly, but the encounter lingers in his mind. Afterward, Preston orders his assistant O’Malley to investigate Alex. He becomes fixated on the realization that her touch affected him when nothing else ever has. Days later, Alex is summoned to the executive floor of Grey Medical Group. She enters Preston’s office and is horrified to realize he is the man from the bar — and her boss. Preston closes the door and confronts her, testing her reactions, invading her personal space. He demands that she repeat the touch from the bar. Alex refuses, uncomfortable and afraid. Preston escalates, offering money in exchange for compliance and asserting that everyone has a price. Humiliated and furious, Alex slaps him across the face and storms out. Instead of anger, Preston experiences intense arousal. The slap confirms his suspicion: Alex is the trigger. He begins to fantasize about surrendering control to her, a notion that terrifies and excites him. Soon after, Alex is framed at the hospital. Shift records are altered to make it appear she neglected a patient. A bereft family member attacks Alex in a hospital corridor, nearly striking her with an IV pole. Preston arrives at the last second, catching the pole mid-swing. He physically shields Alex and de-escalates the situation. He launches an internal investigation, uncovers surveillance footage proving Izzie manipulated the records, and fires her publicly. Izzie is humiliated and removed from the hospital.
