TALENT USAGE Production Type: photoshoot Anticipated Outputs of Production Materials: such as but not limited to static imagery, .GIF files and/or .MP4 files Talent Usage: Entire library of assets Media: All Media; Excluding OOH and broadcast commercials, streaming and/or materials deemed a 'commercial' under the SAG-AFTRA Commercial Contract. Term: 1 Year Global HCP Usage. Starts at the date of first use or 6 months after the shoot Territory: Global Pre-negotiated Renewal Option: detailed below TALENT Exclusivity Option: Exclusivity in the migraine space: $2,500 + 20% per talent AI USAGE Model acknowledges and agrees that materials produced under this agreement (including talent's likeness, image, picture and on- camera performance or portrayal) may be uploaded to an artificial intelligence platform for purposes of creating lifts, edits, and versions of such materials. The artificial intelligence platform that the talent’s assets will be uploaded to is a closed source, meaning that the Model’s likeness will only be used in the AI tool(s) for this specific project/campaign for this specific client. The Model’s likeness will not be used or stored into the AI tool(s) for learning purposes or future projects unless applicable. For the purposes of this project all AI will be generated from still photography, and no live video will be used in the content creation. FIT DAY: May 19, 2026 SHOOT DAY/LOCATION (only one per talent): May 20-21, 2026 HERO TALENT Session Fee: $450+ 20% Agent Fee for 8 hours excluding a 30-minute lunch Usage Fee: $1,200 + 20% Agent Fee Fitting Fee: $250 if applicable Renewals are pre-negotiated for an additional year at the rates below: Photo Renewal Option: 1 Year: $1,500 + 20% 2 Year: $3,000 + 20%
