No redlines will be accepted at time of booking. -------------------------------------------------- ELI LILLY PHARMA NON UNION, STILLS & MOTION SHOOT / APRIL 2026 FIT DATE: April 8 SHOOT DATES: April 9, 10 *Multiple shoot days cannot be guaranteed* Shoot Location: Los Angeles, CA LA LOCALS ONLY RATES: *+20% Agency Fee if applicable ADULTS 18+ YRS FITTING RATE: $250+20% (to include 3 hr fitting). SESSION RATE: $500+20% (to include / per 10 hr shoot day). USAGE RATE: $2,000+20% (to include the below usage - guaranteed at time of booking). EXCLUSIVITY RATE: $2,500+20% (to cover exclusivity category for 1 year - guaranteed at time of booking). USAGE: NON UNION TALENT ONLY TERM: 1 Year from first date of use. TERRITORY: Worldwide/Global (digital/internet worldwide by nature). MEDIA: All Media (excluding broadcast). Entire Library of Assets, Unlimited, unrestricted, exclusive, Worldwide Global, Usage, in all areas of advertising in all media including but not limited to print, brochures, collateral, worldwide web, digital, electronic media, all social and emerging medias and apps, including all outtakes/assets and non-broadcast video including all edits and versions. TALENT CATEGORY EXCLUSIVITY: 1 year from date of first insertion or 90 days from last shoot date, whichever comes first, in the following categories: All of Alzheimer’s including competitors and diagnostics campaigns branded and unbranded from confirmed booking date/contract signing to end date of usage exclusivity term. PRE-NEGOTIATED USAGE OPTIONS (Once the initial term expires. Not guaranteed, up to client’s discretion if picked up): USAGE RENEWAL TERM 2: One (1) Year Renewal Rate Beyond Expiry: $2,200 + 20% to renew for 1 additional year. USAGE RENEWAL TERM 3: Two (2) Years Renewal Rate Beyond Expiry: $4,840 + 20% to renew for 2 additional years. EXCLUSIVITY RENEWAL TERM 2: Additional $2750 + 20% Agency Fee. One (1) Year Renewal Rate Beyond Expiry to renew exclusivity for 1 additional year. EXCLUSIVITY RENEWAL TERM 3: Additional $6050 + 20% Agency Fee. Two (2) Years Renewal Rate Beyond Expiry to renew exclusivity for 2 additional years. Note: In the event talent performance is produced in the United States and should fall within the definition of Performer under the applicable SAG-AFTRA Commercials Contract, talent must agree to be bound by the terms and conditions (including exclusivity) of the applicable Contract and agree to accept scale rates. Further, talent must agree to sign a copy of the standard SAG-AFTRA Contract which Agency will furnish to me. --------------------------------------------------
