SHOOT DATE: JANUARY 9, 2026 FIT DATE: TBD Shoot Location: Indianapolis Talent must work as locals. RATES: *+20% Agency Fee if applicable FITTING RATE: $50 per hour SESSION RATE: $1,000+20% per 10 hour session, to include any and all potential fittings. USAGE RATE: $7,500+20% to include the below term, territory, and media. Payable only if talent appears and is recognizable in the final materials and provided the materials go live. NOTES: - Above fees are for 10 hour shoot day unless otherwise noted. - Fees are exclusive of any agent's commission. 20% agent fee to be paid only if applicable. - Overtime: OT to be billed at 1.5x for 10th & 11th hrs, if applicable - Except as noted here, no special terms, conditions or fees can be granted or negotiated with performer without prior W+K approval. - Compensation covers any and all materials and use of the captured content, in whole or in part, for advertising, trade or any other lawful purpose in any media without restriction during the Term within the Territory. USAGE TERM: 12 months TERRITORY: North America (including United Stated, Canada and Mexico) USAGE/MEDIA: ATL Above The Line MEDIUMS: PRINT/OOH MEDIA All Non-Broadcast Print Media, including: Print/Newspaper, OOH BTL Below The Line MEDIUMS: DIGITAL/INTERNET MEDIA, RETAIL/COLLATERAL/INDUSTRIAL/CORPORATE Unlimited and Unrestricted Digital, Internet, mobile, digital video, banners/paid media, social media, all new electronic media (of any description not yet invented). Also includes Direct Mail, Retail/POP, Collateral, Industrial and paid/nonpaid PR/events. SECONDARY MEDIA: PR / INTERNAL / ARCHIVAL: Earned PR/ Internal/Archival use, worldwide, in perpetuity, including but not limited trade press, textbook, award shows, historical sections of Agency’s and Client’s websites, assets placed on client-owned/agency-owned channels provided materials were placed during Usage Term, and historical/archival/internal use. EXCLUSIVITY: Pharmaceuticals EXCLUSIVITY TERM – From the start of the session, continues for 12 months from first use (first use must commence by/before May 31st, 2026). USAGE OPTIONS If the client opts to use materials for additional Territory/Term below usage option rates will apply: • Option to extend Territory to Global during Term for an additional $7,500 • Option to extend Term (N. America only) use for an additional, consecutive 6 month term for $3750 + 10%. • Option to extend Term (N. America only) use for an additional, consecutive 12 month term for $7500 + 10%. • Option to extend Term (N. America only) for an additional, consecutive 18 month term for $11,000 + 10%. • Option to extend Term (Global) use for an additional, consecutive 6 month term for $7500 + 10%. • Option to extend Term (Global) use for an additional, consecutive 12 month term for $15,000 + 10%. • Option to extend Term (Global) use for an additional, consecutive 18 month term for $22,500 + 10% TALENT PAYMENTS: To be paid by Wieden + Kennedy on behalf of Eli Lilly. Talent Session fees to be paid within 15 business days of shoot. Usage buyouts – if applicable – to be paid within 30 business days
